Entourage effect
The entourage effect is the hypothesis that cannabis's constituent cannabinoids, terpenes, and flavonoids interact synergistically to produce pharmacological effects not predicted from single-compound activity. The term was introduced in an ECS context by Ben-Shabat, Mechoulam et al. (1998, European Journal of Pharmacology 353:23), describing how endogenous fatty acid amides augmented 2-AG activity at cannabinoid receptors. Russo (2011, British Journal of Pharmacology 163:1344) extended the concept to phytocannabinoid–terpene synergy, proposing specific combinations (e.g., CBD + limonene for anxiety, β-caryophyllene + myrcene for analgesia). ⚠️ Critical appraisal: the hypothesis is plausible and some in vitro interactions are documented, but Finlay et al. (2020, Frontiers in Pharmacology 11:359) found no direct modulation of CB1 signaling by six common terpenes at physiologically relevant concentrations, and Cogan (2020) and Christensen et al. (2023) reviewed the human literature and concluded that robust RCT evidence for specific synergies remains sparse. Classification: partially hypothesis-stage.
